We’re seeking an experienced Nurse, Enrolled or Registered, to join a well-established medical practice in the Rockhampton area. This role focuses on empowering patients to manage chronic conditions and improve their overall health outcomes. We can entertain full-time or part-time opportunities.

 

Key Responsibilities

• Prepare Care Plans and Team Care Arrangements

• Conduct health assessments and assist in early identification of chronic disease

• Coach patients on health behaviour change principles

• Collaborate with GPs and a multidisciplinary team to deliver preventative care

• Manage administrative tasks related to Chronic Disease Management appointments

 

About You

• AHPRA registered Enrolled or Registered Nurse

• Experience in Chronic Disease Management and health assessments

• Strong organisational and communication skills

• A friendly, supportive approach and passion for patient care
