We are looking for a Case Manager with NDIS experience !!!

About us

Home Caring is a reputable and fast-growing provider of NDIS and Aged Care services, nationwide. We are currently seeking a passionate and experienced RN Case Manager to join our team and support a growing number of clients in the Auburn/Strathfield and surrounding area.

This role is ideal for someone who is community-minded, culturally sensitive, and committed to providing high-quality, person-centred care. You’ll be working with clients across both Aged Care and Disability sectors.

Key Responsibilities:

  • Developing person-centred care/support plans

  • Coordinating services, monitoring outcomes, and reviewing care plans regularly

  • Advocating for clients and ensuring their cultural, linguistic, and health needs are met

  • Supporting clients to access appropriate services and resources

  • Liaising with families, support workers, and health professionals

  • Ensuring compliance with the NDIS quality standards

  • Enrolling and managing of NDIS funding for Disability clients


What We’re Looking For:

  • Relevant NDIS Case Management experience

  • Experience in aged care, disability, or community health settings

  • Strong communication, problem-solving, and organisational skills

  • Empathy, cultural awareness, and a passion for helping others

  • Current First Aid, CPR and NDIS Worker Screening

  • Driver's Licence and reliable vehicle

  • Full working rights in Australia

  • Ability to assist the growth of the business by sourcing and signing up clients

  • Vietnamese-speaking capability (desirable) to support culturally appropriate service delivery

  • Must be willing to travel to surrounding areas - Banana Shire.
